Bortenat 2 (Bortezomib) 2 mg
Bortenat 2 mg is an antineoplastic agent belonging to the class of proteasome inhibitors. Active ingredient: Bortezomib (2 mg).
Manufacturer: Natco Pharma (India). It is a cost-effective generic version of the brand drug Velcade.
How it works:
Bortenat inhibits the 26S proteasome, a large protein complex that degrades ubiquitinated proteins. By blocking this process, it disrupts cancer cell signaling and leads to cell death (apoptosis), specifically targeting myeloma cells.

Indicated for the treatment of adult patients with:
- Multiple Myeloma: Both for previously untreated patients (in combination with other drugs) and for relapsed/refractory myeloma.
- Mantle Cell Lymphoma: For patients who have received at least one prior therapy.

Routes of Administration:
- 💉 Subcutaneous (SC): Recommended injection into the thigh or abdomen. Associated with lower rates of peripheral neuropathy.
- 💉 Intravenous (IV): Administered as a 3-5 second bolus injection.
Typical Cycle: Twice weekly for two weeks (Days 1, 4, 8, 11) followed by a 10-day rest period (21-day cycle).
Reconstitution: Must be reconstituted with 0.9% Sodium Chloride. Concentration differs for SC vs IV use.
- Hypersensitivity to bortezomib, boron, or mannitol.
- Acute diffuse infiltrative pulmonary and pericardial disease.
- Pregnancy and breastfeeding.
Precaution: Use with caution in patients with hepatic impairment (liver problems) or severe renal impairment.
Common adverse reactions include:
- Peripheral Neuropathy: Numbness, tingling, or burning sensation in hands/feet (less common with SC injection).
- Hematologic: Thrombocytopenia (low platelets), Neutropenia, Anemia.
- Gastrointestinal: Nausea, diarrhea, vomiting, constipation.
- Fatigue and Pyrexia (fever).
