Pemnat (Pemetrexed) 500 mg
Pemnat (Pemetrexed 500 mg) is an antifolate chemotherapy agent used for specific types of lung and lining cancers.
Manufacturer: Natco Pharma (India). A generic equivalent of Alimta.
Mechanism of Action:
Pemetrexed disrupts folate-dependent metabolic processes essential for cell replication. By blocking these pathways, it inhibits the DNA and RNA synthesis in cancer cells, causing them to die.
β Key Benefit: Considered the standard of care for Malignant Pleural Mesothelioma and Non-Squamous NSCLC.
Strictly indicated for:
- π« Non-Small Cell Lung Cancer (NSCLC): Specifically for Non-Squamous histology (e.g., Adenocarcinoma). β οΈ NOT indicated for squamous cell carcinoma.
- ποΈ Malignant Pleural Mesothelioma: Cancer of the lung lining, usually treated in combination with Cisplatin.
Form: Lyophilized powder for IV infusion (500 mg).
β οΈ MANDATORY PREMEDICATION REGIMEN:
To reduce severe toxicity, patients must follow this schedule:
- π Folic Acid: Start taking daily 7 days prior to the first dose and continue throughout treatment.
- π Vitamin B12: One injection every 9 weeks (start 1 week before first dose).
- π Corticosteroids (Dexamethasone): Take the day before, the day of, and the day after infusion to prevent severe skin rash.
- Squamous cell carcinoma histology.
- Pregnancy and breastfeeding.
- Severe renal impairment (Creatinine clearance < 45 ml/min).
- Concomitant yellow fever vaccine.
Common side effects (mitigated by vitamins):
- π Bone Marrow Suppression: Fatigue, anemia, neutropenia (low white blood cells).
- π€’ Gastrointestinal: Nausea, vomiting, stomatitis/mucositis.
- π΄ Skin: Rash or desquamation (if corticosteroids are skipped).
