Tipanat 20 (Trifluridine / Tipiracil) is a combination oral chemotherapy drug.
Manufacturer: Natco Pharma (India). Tipanat is a cost-effective generic version of the brand-name drug Lonsurf.
Mechanism of Action ("The Attacker and The Protector"):
It consists of two active ingredients. Trifluridine is the "attacker": it incorporates directly into the cancer cell's DNA, preventing it from multiplying. Tipiracil acts as the "protector": it inhibits the enzyme that usually breaks down Trifluridine in the body, maintaining high enough levels of the drug in the blood to effectively fight the tumor.

Approved for the treatment of adult patients with:
- ๐๏ธ Metastatic Colorectal Cancer (mCRC): Who have been previously treated with standard chemotherapy (fluoropyrimidine, oxaliplatin, irinotecan).
- ๐๏ธ Metastatic Gastric Cancer: Including adenocarcinoma of the gastroesophageal junction, after at least two prior lines of chemotherapy.

โ ๏ธ DOSING SCHEDULE (28-Day Cycle):
- ๐งฎ Calculation: The dose is strictly calculated based on Body Surface Area (BSA).
- ๐ Regimen: Typically taken twice daily on Days 1 through 5 and Days 8 through 12 of each 28-day cycle.
- ๐ฝ๏ธ With Food: Must be taken within 1 hour after completion of morning and evening meals. Taking it on an empty stomach reduces effectiveness. Swallow whole with water.
- Severe renal or hepatic impairment.
- Pregnancy and breastfeeding (embryo-fetal toxicity).
- Hypersensitivity to Trifluridine or Tipiracil.
Monitoring is crucial due to bone marrow suppression:
- ๐ Myelosuppression: Severe neutropenia (risk of sepsis), anemia, and thrombocytopenia. Complete blood counts (CBC) must be monitored prior to and during the cycle.
- ๐คข Gastrointestinal: Nausea, vomiting, decreased appetite, diarrhea.
- ๐ด Fatigue: Asthenia and extreme tiredness are very common.
